Order Callback

1.    Display the corresponding service authorization form by pressing: ClearWorld Callback form or World Access Callback  form

2.    Then from the FILE/PRINT Menu, PRINT the form when it is displayed.

3.    IMPORTANT: The form is designed to be printed on a single page. If you are unable to print the form neatly ON A SINGLE PAGE, please e-mail us your fax number or mailing address and we will send you a pre-printed form.

4.    Complete the form with your name, address etc.Callback Numbers: Your telephone number(s) where you are going to call from and where the Computer is going to call back to.

5.    When you have completed and signed the printed form, send it to us:

6.    Do NOT return form by e-mail. Your signature is required.

7.    Order will be ready in 5 business days.
